The CVC Program is an initiative from the Department of Veterans’ Affairs (DVA) focused on preventative care. Its goal is simple: to help veterans stay healthier for longer by providing regular monitoring, early intervention, and better coordination between healthcare providers.
Rather than only treating issues as they arise, the program takes a proactive approach. It supports veterans in managing their health through ongoing care planning, helping to reduce the risk of complications and hospital visits over time.
Through the CVC Program, eligible veterans receive a personalised care plan tailored to their individual health needs. This often includes regular check-ins, tracking of key health indicators, and support in managing conditions such as heart disease, diabetes, respiratory conditions, and other chronic illnesses.
A key part of the program is coordination. Your healthcare team works together to ensure nothing falls through the cracks, whether that’s monitoring changes in your health, adjusting treatment plans, or connecting you with additional support services when needed.
The program also places a strong focus on early detection. By identifying potential health concerns sooner, it becomes much easier to manage them effectively and avoid more serious complications down the track.

At the Veterans & Families Centre, the CVC Program can be accessed directly without the need for a referral. The process is simple. Veterans can call the centre to book an appointment and let the team know it is for the CVC Program. This ensures the appointment is scheduled first with the practice nurse, followed by the doctor.
Patients will be asked to complete a registration form either before their appointment or on the day. From there, the team will guide them through the next steps and provide ongoing support as part of their care.
